Bluegrass Health Network (BHN) is now providing Disease Management/Health Awareness services for clients with chronic pain, high risk pregnancy, high blood pressure, high cholesterol, coronary artery disease, diabetes, chronic obstructive pulmonary disease and asthma. Our Program has been developed utilizing Milliman Care Guidelines TM, evidence-based decision support and effective patient education materials. BHN’s experienced team of physicians, RNs and case managers are prepared to assist our clients with obtaining the most up to date health information to empower and motivate them to actively participate in the management of their chronic health condition.
